LICENSED COMMUNITY RESIDENCE
PROGRAMS
Programs are in congregate (group home) and apartment living settings
and provide room, board and supervision. The overall goals of a
community residence program are to help an individual learn the
necessary skills to live independently in the community and to attain
emotional stability. These programs are rehabilitative and transitional.
Community residences have a maximum of 24 individuals in any one
location. Supported apartments provide the least restrictive living
environments with needed daily support at program sites. Services are
generally paid for with funding from SSI, SSD, Monroe County Public
Assistance and Medicaid. Community residential programs are accessed
through the Monroe County Single Point of Access (SPOA) and the
application can be found on-line at www.carecoordination.org or by
calling SPOA at 753-2879.

FAMILY CARE HOMES
These settings provide a homelike atmosphere for individuals with
mental health disabilities by enabling them to live with an intact family
in the community. These persons are under the supervision of the state,
and are provided with medical and psychiatric follow-up. Homes and
caretakers are carefully screened and trained. The homes are supervised,
monitored and must adhere to set standards. Family Care Homes are
located throughout Monroe, Livingston, Genesee, Orleans and Wyoming
Counties.

SINGLE ROOM OCCUPANCY PROGRAMS
Extended or permanent stay residences with private living units and onsite
services designed to flexibly meet the needs of individuals with
Serious and Persistent Mental Illness (SPMI). These might include 24-
hour staffing, and assistance with activities of daily living, case
management, medication management, social/recreation and
housekeeping.

SUPPORTED HOUSING
The objective of the supported housing program is to assist consumers in
locating and securing mainstream housing of their choice and in
accessing the supports necessary to live successfully in the community.
Services may include assistance with choosing housing, roommates and
furniture; help with initial and ongoing financing; and linkage with a
comprehensive community support system of case management, mental
health, rehabilitation, respite, social, employment and health supports.
