MHA is proud to partner with WeLinkCare by FLIPA, helping Medicaid members in the Finger Lakes region access vital social care services—like food, housing, and transportation—through a simple screening process.
What We Offer
- Trained staff to administer the “NYS Accountable Health Communities (AHC) Health-Related Social Needs Screening (HRSN) tool” and determine if there are any unmet Health-Related Social Needs.
- Upon confirmation of an unmet health-related social care need, connection with a MHA Rochester Social Care Navigator to determine eligibility for Health-Related Social Need (HRSN) Enhanced services.
- Comprehensive knowledge of community resources, referrals to needed services and support during the referral process.
- Dedicated follow up on referrals made for health-related social care needs.
- Collaboration with an individual’s care team to facilitate integration and coordination of services.
